Insurance Information All Students of Sydney University and non students who are members of Sydney University Sport and Fitness are covered under the same insurance policy. Both students and non-students need to go through the same process of reporting and claiming injuries. This process is outlined below. Students and non-students do, however, need to contact two different divisions with their forms and claims. Please make sure you are in contact with your correct representative when you are dealing with personal accident claims.
Student Contact Vlad Nesic General Insurance and Claims Officer 9351 4127 v.nesic@usyd.edu.au
Non-student Contact Greg Taylor Finance and Administration Sydney Uni Sport & Fitness 9351 8762 g.taylor@sport.usyd.edu.au
How does it work? IMMEDIATELY REPORT ANY INJURY As soon as you are injured you need to fill out two forms. These both need to be completed by you and received by your insurance rep within 30 days of your injury, so it is best to do them as soon as possible.
When you are injured and get medical treatment, be sure to obtain and keep all receipts of treatment that you pay for. You will need these receipts to attempt to claim back the money that you have paid.
The first form is an Incident Report form. This can be retrieved from http://www.usyd.edu.au/ohs/docs/forms/incident.pdf
The second form is an insurance claim form from the AFA, the university’s chosen insurance provider. The form can be retrieved from http://www.usyd.edu.au/ohs/report/index.shtml or by following the links in the insurance section in http://www.susf.com.au/page/administration.html
This form details several things, including what type of claim you’re making, your details, details of the injury, your current insurance, medical details and expenses that you are claiming. It is important to fill this form in and send it to your insurance contact at the uni even if you are not claiming any expenses yet. You may need to claim for physio, or other treatments in the future, which will be covered by this. If you do not put the form in within 30 days, the insurer can reserve the right to refuse you cover.
This form will also need to be filled out by a ‘registered medical practitioner’. This should be a doctor, as some claims will be denied even if they are from a physio or similar. Someone on the SUANFC committee will also need to fill out the final section.
Once you have filled out both the incident report form and the AFA Claim form, combine originals of all your receipts with the AFA form and send it to your university insurance contact. They will contact you with all the details of your claim and any other actions that you need to take.
What am I covered for? - You are NOT covered for any treatment that is wholly or partially covered by Medicare. - You ARE covered for physiotherapy or similar treatments (in most cases) - up to a value of $3000. If you have private health insurance, the uni cover will cover the gap between the private health insurance cover and the total cost. - You can be covered for loss of income to an extent (to a maximum of $250.00 per week for a maximum of 26 weeks). - Hospitalisation fees (not doctors) - Dental treatment - Ambulance costs
If you are covered by private health insurance, you must first claim from this.
